    The Registered Nurse is responsible to the Unit RN Supervisor, and is to be clinically competent; delivers care to patients utilizing the nursing process of assessment, planning, intervention, implementation, and evaluation; and effectively interacts with patients, significant others, and other health care team members while maintaining standards of professional nursing.

    Responsibilities

     

    *Complies with PENN HIGHLANDS BROOKVILLE  policies and procedures, accreditation agency requirements, and federal, state or local law and regulations

     

    Obtains health information through interview process, assessment, and data collection

    Collaborates with multidisciplinary team regarding nursing assessment findings and plan of care

    Identifies patient problems/focus through physical assessment, interview, and observation

    Implements nursing care in accordance with BrookvilleHospital standards of care, policies and procedures

    Evaluates patient's response to medical, nursing, and other disciplines’ interventions

    Revises the plan of care with input from the patient, significant other, and other health care providers

    Utilizes clinical and social resources

    Administers medications according to policy/procedure/protocol {i.e. patient ID, follows 5 R’s, etc.}

    Maintains proficiency in starting and maintaining IV lines utilizing standard precautions

    Responds appropriately to changes in patient condition

    Reviews medical record for diagnostic test results on an ongoing basis

    Conducts clinical rounds on patient assignment after receiving report

    Implements the plan of care according to physician’s, nursing orders, nursing care standards, and multidisciplinary team decision/recommendations

    Identifies patient’s biopsychosocial and learning needs on admission and throughout a patient's stay

    Initiates discharge planning and collaborates with other multidisciplinary team members to achieve goals

    Collaboratively develops and implements a teaching plan for discharge

     

    Evaluates the results of patient teaching and discharge planning

    Coordinates the written instruction provided to patients/significant others regarding medications, activities, diet, treatment, and follow-up plan of care with patients and/or significant others and documents accordingly

    Provides didactic instruction on health and medication topics

    In collaboration with multidisciplinary team, plans patient care based on the priority of needs and available resources

    Delegates and supervises assignment to other staff members according to patient needs and abilities of personnel

    Reports changes in patient's condition to physician and nursing personnel and follows-up

    Conducts a shift summary by reporting on assigned patients

    Performs responsibilities for patient care/unit operation

     

    Monitors patient's response to interventions performed by other staff members

    Utilizes cost effective measures in delivering patient care

    Monitors the environment of care and implements measures to maintain safety of staff, patients, and visitors

    Supports and demonstrates understanding of mission and goals of the unit

    Facilitates the admission process based on efficiency, effectiveness and customer service

    Orients and/or preceptors new staff

     

    Demonstrates welcoming behavior to new employees

    Introduces new staff to all health care team members

    Assures socialization of new staff (i.e. take along to lunch, breaks, and educational programs)

    Serves as resource person for new staff

     

    Participates in the development of an individualized orientation plan that is based on the new staff member's education, experience, competence, and unit priorities

    Utilizes educational resources as indicated

     

    Follows unit-based performance standards as criteria for evaluation of new staff

    Participates in the evaluation process of new staff members on a regular basis

    When functioning as a preceptor/mentor, submits a written progress report on new staff members to the director/manager as per unit protocol

    Complies with departmental and hospital polices regarding attendance and work schedule

    Participates in the development and revision of competency-based standards of care and unit based goals and objectives

    Attends in-service programs on new products/equipment and procedures

    Presents information obtained at education conferences to staff members

    Participates in the improvement of department through individual or work team contributions

    Provides feedback to enhance coworker performance

    Participates in unit staff meetings as scheduled

     

    Complies with departmental and unit policies regarding dress code

    Develops and promotes professional relationships

    Utilizes appropriate resources for problem resolution

    Solves those problems that are within their scope of responsibility and practice

    Informs Unit RN Supervisor/Program Manager of unusual or difficult patient care problems and consults with ANC Supervisor Unit as needed

    Appropriately communicates evaluation of patient response to intervention to the multidisciplinary team

    Performs patient safety/observation checks every 15 minutes

    Complies with contraband policy

     

    Manages changes in behavior that puts the patient or others at risk

    Complies with restraint/seclusion policies

     

    General duties as directed.

     

     

     

     

     

    Qualifications

    Current Pennsylvania Registered Nurse license or Temporary Practice Permit required

    • Previous behavioral health experience preferred
    • Comprehensive Crisis Management training (or must obtain within one year of start date) required
